Monday–Friday 9:00 AM–6:00 PM: appointment only; no walk-ins. Saturday 10:00 AM–3:00 PM and Sunday 11:00 AM–3:00 PM: appointments and walk-ins welcome.

Bring your prepaid EV Screening authorization and photo ID. EV Screening uses the laboratory’s paper chain-of-custody (COC) form for specimen collection and shipping. Your EV authorization/voucher confirms prepaid EV Screening service; it is not a laboratory COC or laboratory order.
