Employer testing, from request to collection

1. Tell us what you need. Share the service, estimated number of donors, location, and preferred dates. We review the account application, authorized ordering and billing contacts, and separate Employer Service Agreement signed through DocuSign before enabling employer ordering.

2. Review one order. Select Non-DOT urine, hair, or nail testing and confirm the donor count, pricing, and one combined prepaid checkout. Donor details can be provided individually; roster or CSV upload is not currently available. An approved account can request repeat-order help. After approximately 60 days in good standing, an employer may be considered for Net-15 billing, subject to EV Screening approval; it is not automatic.

3. Send donors with prepaid authorizations. Each purchased test receives its own single-use authorization. The donor presents the code and ID. An EV authorization is proof of prepaid service, not a laboratory chain-of-custody form or eCOC. The appropriate laboratory electronic order is created at donor arrival.

Volume pricing

1–9 tests: standard price. 10–19: 10% off. 20–29: 15% off. 30 or more: 20% off. Ask about EV Screening’s Price Match or Beat Program by sending a current/recent qualifying invoice or pricing for review. Terms and eligibility apply. Price Match or Beat, volume discounts, coupons, negotiated pricing, and other promotional pricing do not automatically stack. Ask for a written quote for mobile collections or unusual needs.
